PALUSO, COMMISSIONERS Minute No. 365 Washington County Commissioners Washington, PA, September 19, 1991 The Board of Washington County Commissioners met in regular session in the Public Meeting Room, Courthouse Square, Washington, Pennsylvania, with the following members being present: Commissioners Mascara, Petrosky and Paluso. Also being present: Chief Clerk Dallatore; Solicitor Bassi; Coroner Jackson; Duane Kavka, Director of Human Services; Tom Vizacaro, Diversified Health Services; Dave Suski, JTPA Director; Linda Hands, Clerk -Steno II; Bill Prentice, Property and Supplies; Jon Stevens, Observer -Reporter; Cathy Caldwell, WKEG; Jim Jefferson, WJPA; and Ken Nowak of Automated Entrance Systems Company, Inc. Chairman Mascara called the meeting to order at approximately 10:00 a.m. and entertained a motion to approve Minute No. 364. Moved by Mr. Paluso, seconded by Mr. Petrosky, approving Minute No. 364 as written. Roll call vote taken: Mr. Paluso - Yes; Mr. Petrosky - Yes; Mr. Mascara - Yes. Motion carried unanimously. OLD BUSINESS Solicitor Bassi recommended that the bids for Medical Supplies for the period of October 1, 1991 through March 31, 1992, be awarded to the following vendors for the per unit costs submitted in their bid proposals: Calgon Vestal Laboratories, St. Louis, MO Gatti Medical Supply, Indiana, PA General Medical Corporation, Erie, PA Hollister, Inc., Libertyville, IL Medline Industries, Pittsburgh, PA Penn Medical Supply, McMurray, PA Randolph Medical, Inc., Cleveland, OH Red Line Medical Supply, Golden Valley, MN Stuart's Drug & Surgical Supply, Inc., Greensburg, PA Moved by Mr. Petrosky, seconded by Mr. Paluso, awarding the bids as per above; these being the lowest responsible bids received in accordance with specifications; and authorizing the Chief Clerk to re -advertise for bids for certain items. REMARKS: It was noted that certain items were not awarded to the lowest bidder as no samples were submitted for evaluation or samples submitted to the Washington County Health Center were unacceptable. Roll call vote taken: Mr. Paluso - Yes; Mr. Petrosky - Yes; Mr. Mascara - Yes. Motion carried unanimously, Systems 75's for a one-year period be awarded to AT&T of Pittsburgh, Pennsylvania, based on the per unit costs and review of the Maintenance Contract by the County Solicitor and the County Communications Manager. Total award will be based on an inventory of equipment by the County and AT&T - approximate cost $12,000.00). Moved by Mr. Petrosky, seconded by Mr. Paluso, awarding the bid as per above; this being the only bid received in accordance with specifications. Roll call vote taken: Mr. Paluso - Yes; Mr. Petrosky - Yes; Mr. Mascara - Yes. Motion carried unanimously. Solicitor Bassi recommended that the bids for Food and Staples for the Washington County Health Center and Jail for the period of October 1, 1991 through March 31, 1992, be awarded to the following vendors for the per unit costs submitted in their bid proposals: Bernard Food Industries, Valencia, PA Cameron Coca-Cola, Washington, PA Colteryahn Dairy, Pittsburgh, PA Deaktor/Sysco, Harmony, PA Jarrell & Rea, Pittsburgh, PA Kaufmann's Bakery, Inc., Pittsburgh, PA Kraft Foods, Bridgeville, PA Schneider's Dairy, Washington, PA John Sexton & Company, Pittsburgh, PA Moved by Mr. Petrosky, seconded by Mr. Paluso, awarding the bids as per above; these being the lowest responsible bids received in accordance with specifications. Solicitor Bassi recommended that the bids for Meat and Produce for the month of October be awarded to the following vendors for the per unit costs submitted in their bid proposals: Meat: Green Valley Packing Company, Claysville, PA Kraft/Behrhorst, Bridgeville, PA Deaktor/Sysco, Harmony, PA Jarrell & Rea, Pittsburgh, PA Tri-River Foods, Bethel Park, PA Produce: Total award to Kraft/Behrhorst, Bridgeville, PA, being the only bid received for produce. above: these being the lowest responsible bids received in accordance with specifications. Roll call vote taken: Mr. Paluso - Yes; Mr. Petrosky - Yes; Mr. Mascara - Yes. Motion carried unanimously. NEW BUSINESS Chief Clerk presented to the Board for their consideration and approval an Agreement with Diversified Health Services of Plymouth Meeting, Pennsylvania, for the purpose of implementing a Quality Assurance Program at the Washington County Health Center. The costs would be as follows: (a) $25,000.00 maximum to implement the Quality Assurance Program; and (b) $5,000.00 per month (for a period of up to two years) for on -going quality assurance monitoring. Moved by Mr. Paluso, seconded by Mr. Petrosky, approving the Agreement as per above, contingent upon final review and approval by the Solicitor's Office. Roll call vote taken: Mr. Paluso - Yes; Mr. Petrosky - Yes; Mr. Mascara - Yes. Motion carried unanimously. Chief Clerk presented to the Board for their consideration and approval a request for the JTPA Office to submit an application to the State Bureau of Employment Services and Training for funds to provide needs based payments to Title III-EDWAAA participants. The amount of the proposal is $10,790.00 and will serve five participants. The term of the agreement will be from September 1, 1991 through June 30, 1992. Moved by Mr. Petrosky, seconded by Mr. Paluso, approving submission of the application as per above. Roll call vote taken: Mr. Paluso - Yes; Mr. Petrosky - Yes; Mr. Mascara - Yes. Motion carried unanimously. Chief Clerk presented to the Board for their consideration and approval a recommendation to approve the following Board/Committee appointments: per above. Washington -Greene Drug and Alcohol Planning Commission Appointment of Louis Waller, III, of Washington 3-year term, effective October 1, 1991 Emergency Medical Service Institute (EMSI) Appointment of David Smith, Emergency Management Communications Officer, to serve as the County's representative on the EMS Communications Committee Moved by Mr. Paluso, seconded by Mr. Petrosky, approving the appointments as Roll call vote taken: Mr. Paluso - Yes; Mr. Petrosky - Yes; Mr. Mascara - Yes. PALUSO, COMMISSIONERS Chief Clerk presented to the Board for their consideration and approval Change Order No. 2 to the contract with R. G. Friday Masonry, Inc., relative to the WCHC Lintel Repair Project - Phase II. The Change Order increases the contract amount by $5,800.00 and is for additional repairs to the solarium. Moved by Mr. Petrosky, seconded by Mr. Paluso, approving the Change Order as per above and authorizing the Chairman of the Board to so execute. Roll call vote taken: Mr. Paluso - Yes; Mr. Petrosky - Yes; Mr. Mascara - Yes. Motion carried unanimously. Chief Clerk presented to the Board for their consideration and approval a Service Agreement with Bell Atlantic Network Services, Inc., for the purpose of recovering costs from the Judicial Information System. The initial start-up costs to the County will be approximately $2,700.00. The charges to end users will include a one time set-up fee of $20.00 and per minute use charges of $1.00 per minute for the first minute and $0.60 for each additional minute. Moved by Mr. Petrosky, seconded by Mr. Paluso, approving the Service Agreement as per above and authorizing the Chairman of the Board to so execute. Laura Borish of Children and Youth to attend a Three Rivers (Reimbursable) Adoption Council Meeting in Pittsburgh, Pa., on September 20, 1991 (0) 3. Jackie Forte, Bernie Tychinski and Marcia Wilson of Children and (Reimbursable) Youth to attend the Independent Living Coordinators' Regional Meeting in Greensburg, Pa., on September 25, 1991 (0) 4. Mary Jo Closser and John Patrick of Children and Youth to attend (Reimbursable) the Meeting of Pennsylvania Association of Children and Youth Workers in New Castle, Pa., on October 4, 1991 (0) 5. George Krcelich of Human Services to attend the NACo Employment (Reimbursable) Policy & Human Services Conference in Atlanta, Georgia, on November 16-19, 1991 (0) 6. Catherine Hansen of LMA Day Care to attend the Western Region PACCA (Reimbursable) Meeting in Butler, Pa., on September 20, 1991 (0) 7. Bonnie Levine of the Health Center to attend an Assuring Quality (Reimbursable) in Long Term Care Conference sponsored by the New York Association of Homes and Services for the Aging in Rochester, New York, on December 6, 1991 (0) Optional Moved by Mr. Petrosky, seconded by Mr. Paluso, approving the foregoing travel requests and authorizing payment of all necessary expenses related thereto. MASCARA, METRO PETROSKY, JR, EDWARD M. PALUSO, COMMISSIONERS Chief Clerk presented to the Board for their consideration and approval an Addendum to the Agreement with Community Care, Inc., for the provision of nursing services at the Washington County Health Center. The Addendum provides for the following service rates, effective September 10, 1991: above. LPN $24.50 per hour RN $34.50 per hour Certified Nursing Assistant $14.50 per hour Moved by Mr. Petrosky, seconded by Mr. Paluso, approving the Addendum as per Roll call vote taken: Mr. Paluso - Yes; Mr. Petrosky - Yes; Mr. Mascara - Yes. Motion carried unanimously. ANNOUNCEMENTS Chief Clerk announced that a Salary Board Meeting will be held on September 26th. BID OPENINGS Controller Belcastro opened and read the following bid: HANDICAPPED OPERATED AUTOMATIC DOOR SYSTEM FOR THE WASHINGTON COUNTY COURTHOUSE SQUARE OFFICE BUILDING 1. Automated Entrance Systems Company, Inc. 313 Archie Street Oakmont, PA 15139 Total amount of bid: $23,999.00 A bid bond in the amount of 10$ accompanied the bid. Moved by Mr. Petrosky, seconded by Mr. Paluso, that the foregoing bid be tabled for further review. Roll call vote taken: Mr. Paluso - Yes; Mr. Petrosky - Yes; Mr. Mascara - Yes.
