Chief Clerk, on behalf of the Redevelopment Authority of the County of Washington, respectfully
requests the Commissioners adopt a resolution approving the Consolidated Plan for FYs 2025-
2029, the Action Plan for FY 2025, and amend the Citizen Participation Plan for the Community
Development Block Grant (CDBG), the Home Investment Partnerships (Home) and the
Emergency Solutions Grant (ESG) programs.
Chief Clerk, on behalf of the Redevelopment Authority of the County of Washington, respectfully
requests the Commissioners approve a Cooperation Agreement between the County of Washington
and the Redevelopment Authority of the County of Washington in connection with the
Redevelopment Assistance Capital Program (RCAP) grant in the amount of$2,500,000 for the
Brockway Glass Plant project in Canton Township, in Washington County. No county funds are
required.
Chief Clerk, on behalf of the Redevelopment Authority of the County of Washington, respectfully
requests the Commissioners approve a Cooperation Agreement between the County of
Washington, the Redevelopment Authority of the County of Washington, and Chapman Properties
for the Washington Mall demolition project, No. BMP 25-02 in South Strabane Township,
Washington County, Pennsylvania, under the Washington County Blight Mitigation and
Demolition Program. The County will provide $5,917,075 to be utilized by the Redevelopment
Authority to oversee and complete the work at the property.

Children and Youth Services is respectfully requesting approval of the following provider
agreement with Family Pathways to provide continued foster care placement for the period
of July 1, 2024—June 30, 2025. The contract authorization amount is for$50,000.00 and no
additional county funds are required.
2. Children and Youth Services—Provider Agreement
Children and Youth Services is respectfully requesting approval of the following provider
agreement with Donnelly-Boland and Associates to provide fiscal related services for the
period of May 20, 2025, through June 30, 2026. The contract authorization amount is for
$379,750.00 and no additional county funds are required.
3. Children and Youth Services—Provider Agreement Addendum
Children and Youth Services is respectfully requesting approval of a provider agreement
addendum with KidsPeace National Centers, Inc. for the provision of residential placement
and therapeutic services for the period of July 1, 2024 -June 30, 2025. The contract
authorization amount will be increased from $200,000.00 to $500,000.00 and no additional
county funds are required.
4. Children and Youth Services—Provider Agreement Addendum
Children and Youth Services is respectfully requesting approval of a provider agreement
addendum with Merakey—Pennsylvania for the provision of community based foster care
services for the period of July 1, 2024 -June 30, 2025. The contract authorization amount
will be increased from $100,000.00 to $150,000.00 and no additional county funds are
required.
5. Children and Youth Services—Provider Agreement Addendum
Children and Youth Services is respectfully requesting approval of a provider agreement
addendum with New Life Adolescents,LLC for the provision of residential treatment
services for at risk female youth for the period of December 1, 2024-June 30, 2025. The
contract authorization amount will be increased from$150,000.00 to $275,000.00 and no
additional county funds are required.
6. Behavioral Health and Developmental Services—Provider Agreements
Behavioral Health and Developmental Services is respectfully requesting approval of the

7. Aging Services—Provider Modification Agreement
Aging Services is respectfully requesting approval of a Modification Agreement with
Southwestern Pennsylvania Area Agency on Aging to receive funding for Senior Center
Services, Care Management and Ombudsman Services for the period of July 1, 2025,
through June 30, 2026. The contract authorization amount is for$1,317,962.00. The county
match is up to $1,329,150.00.
8. Aging Services—Provider Agreement
Aging Services is respectfully requesting approval of the following provider agreement with
Dr.John D. Six for Substance Abuse Prevention Presentations for Aging Services. This
agreement is for the period of August 2025 through November 2025;renewable for one
additional year. The contract authorization amount is for$6,000.00 and no county funds are
required.
9. Aging Services—Provider Agreement
Aging Services is respectfully requesting approval of the following provider agreement with
Washington Drug and Alcohol Commission for Prescription Medication Safety Program
Presentations to Senior Centers. This agreement is for the period of July 15, 2025, through
December 31, 2026;renewable for one additional year. The contract authorization amount is
for$5,586.00 and no county funds are required.
10. Human Services—Emergency Shelter Provider Agreement
Human Services is respectfully requesting approval of a provider agreement with SPHS
Connect, Inc. for emergency shelter services for the period of July 1, 2025, through June 30,
2026. The contract authorization amount is for up to $20,000.00. The funds are provided by
the Department of Community and Economic Development (DCED) and no county funds
are required.
11. Human Services—Provider Agreement
Human Services is respectfully requesting the approval of the following provider agreement
with the Redevelopment Authority of the County of Washington (RACW) for the Home-
ARP Program for housing stability funds to include housing case management and financial
assistance for the period of June 19, 2025, through June 30, 2027,with the opportunity to
renew the contract for three additional one-year periods. The contract authorization amount
is for$916,709.90 and no county funds are required.
12. Human Services—Provider Agreement
Human Services is respectfully requesting approval of a provider agreement with Catholic
Charities,Diocese of Pittsburgh for rapid rehousing services for the period of July 1, 2025,
through June 30, 2026. The contract authorization amount is for up to $65,000.00 and the
county will provide a one-hundred percent match.
13. Human Services—Master Grant Agreement Addendum
Human Services is respectfully requesting approval of an agreement addendum with the
United States Department of Housing and Urban Development (HUD) for the Rapid
Rehousing Program. The purpose of the addendum is to extend the period of performance
by one month, from May 31, 2025, to June 30, 2025. No county funds are required.
14. Human Services—Grant Application
The Department of Human Services is respectfully requesting permission to apply for the
Department of Community and Economic Development (DCED) Emergency Solutions Grant
to support the existing emergency shelter and rapid rehousing efforts. One-hundred percent
County match is required.

2.Ford Business-Maintenance and Service Agreement
Purchasing, on behalf of Information Technology,respectfully requests the Board of County
Commissioners rescind the award for a monthly maintenance and service agreement with
ComDoc,Inc.,a Xerox Company as approved on MIN#1205,Dated 04/03/2025 and
approve a monthly maintenance and service agreement with Ford Business Machines,Inc.,
Connellsville,PA, for parts,toner, drums and labor for approximately 165 County office
printers.
If approved,the total monthly maintenance cost will be $3,184.50 and shall continue for 12
months from the date of approval,with annual options to renew unless terminated pursuant to
written notice.
3.WCCF—Addendum-Lawrence County Agreement
Purchasing, on behalf of Warden Fewell, respectfully requests the Board of County
Commissioners approve an addendum to our agreement with Lawrence County for the
provision of housing each other's adult offenders on an as needed basis as per the contract
conditions including, $75.00 per inmate,per day,plus medical expenses to be paid from their
Operating budget.

4. SynergIt VMware Subscription Licenses
Purchasing, on behalf of Public Safety,respectfully requests the Board of County
Commissioners approve the renewal of the 128 VMware subscription licenses to provide
support for the HP blades, servers,and SAN systems located in our 911 Center with
SynergIT.
The cost of the licenses for a 1-year period is($50/each) $6,400 per year.
5.HVAC Maintenance—Family Court Center—Building Control Systems,Inc.
Purchasing, on behalf of Justin Welsh, Director of Building& Grounds,respectfully
requests the Board of County Commissioners extend our current agreement with Building
Control Systems Inc., for preventative maintenance for the HVAC Temperature Control
System for the Family Court Center.
If approved,this contract extension will be effective August 1, 2025,through July 31, 2026,
with an option to extend year to year. Total cost is $4,795.00. All repairs will be done"as
needed"at the rate of$150/hr.regular time and parts will include a multiplier/percent markup
against the published list price. This contract extension is under the same terms and
conditions as the original contract.
6. Lease Agreement-Robert P.Bott,DMD
Purchasing,respectfully requests the Board of County Commissioners approve a lease
agreement between Washington County and Robert P.Bott,DMD,9 Campbell Street,
Avella for a period of one(1)year,with options to extend at a monthly rental rate of$280.00.

1. Office Furniture Donation
The Purchasing Department,as per Section 1805 of the County Code governing the sale or
disposition of personal property of the County,respectfully requests the Board of County
Commissioners authorize the resolution for the donation of office furniture from
Washington County to be utilized by the Loyal Inc.,who has completed the"Application
for Donation"application and have met all required criteria.
2.LED Lights Donation
The Purchasing Department,as per Section 1805 of the County Code governing the sale or
disposition of personal property of the County,respectfully requests the Board of County
Commissioners authorize the resolution for the donation of LED Lights from Washington
County to be utilized by the Valley Inn VFD who has completed the"Application for
Donation"application and have met all required criteria.

The Purchasing Department, as per Section 1805 of the County Code governing the sale or

FY 2025-29 CONSOLIDATED PLAN, FY 2025, ANNUAL ACTION PLAN, AND CITIZEN
PARTICIPATION PLAN
RACW requests that the Board of Commissioners adopt a resolution to approve the Washington County
FY 2025-2029 Consolidated Plan, FY 2025 Action Plan, and the Citizen Participation Plan. The
Consolidated Plan is a five-year plan to strategically implement the federal programs that fund housing,
community development, and economic development activities in the County. The Action Plan serves
as a funding application that is submitted to the U.S. Department of Housing and Urban Development
for three formula grant programs including the Community Development Block Grant(CDBG)Program
at$3,425,458,the Home Investment Partnerships (HOME) Program at$572,270.43, and the Emergency
Solutions Grants (ESG) Program at $299,886. The revisions to the County Citizen Participation Plan
updates the Redevelopment Authority's address as well as public display locations and the definition of
a substantial amendment.
These Plans were on public display from May 12 through June 10, 2025. No comments were received
during the public comment period. RACW administers the CDBG and HOME Programs while the
County Department of Human Services administers the ESG Program. The overall goal of the
community planning and development programs covered by the Consolidated Plan is to develop viable
urban communities by providing decent housing,a suitable living environment and expanding economic
opportunities, principally for low- and moderate-income persons.

LeMoyne Community Center Renovation Project No. LS 23-12
This project involves the construction of an addition to the front of the LeMoyne Community Center
located at 200 North Forrest Avenue, Washington. The addition will include two new classrooms and
restroom facilities to the right of the existing entrance and expansion of the existing office space to the
left of the front entrance. A new expanded foyer will also be created with a rounded portico featuring
new signage for the Center.

RESOLUTION OF THE BOARD OF COMMISSIONERS OF THE COUNTY OF
WASHINGTON APPROVING THE FY 2025-2029 CONSOLIDATED PLAN, FY 2025
ACTION PLAN AND AMENDED CITIZEN PARTICIPATION PLAN FOR THE
COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG) PROGRAM,
HOME INVESTMENT PARTNERSHIPS (HOME) PROGRAM, AND
EMERGENCY SOLUTIONS GRANTS (ESG) PROGRAM
WHEREAS,under Title I of the Housing and Community Development Act of 1974, as amended,
the Secretary of the U.S. Department of Housing and Urban Development is authorized to extend
financial assistance to communities in the elimination or prevention of slums or urban blight, or
activities which will benefit low- and moderate-income persons or other urgent community
development needs; and
WHEREAS,under the Home Investment Partnerships Program created by the National Affordable
Housing Act of 1990 (NAHA) as amended by the Housing and Community Development Act of
.1992, the Secretary of the U.S. Department of Housing and Urban Development is authorized to
extend financial assistance to participating jurisdictions to expand the supply of decent, safe,
sanitary and affordable housing; and
WHEREAS, under the Emergency Solutions Grants Program (ESG) contained in the Stewart B.
McKinney Homeless Assistance Act of 1988, as amended through the Homeless Emergency
Assistance and Rapid Transition to Housing Act of 2009 (HEARTH Act),the Secretary of the U.S.
Department of Housing and Urban Development is authorized to extend financial assistance to
grantees and non-profit agencies which provide essential services, operations, street outreach,
homeless prevention,rapid re-housing,and shelter rehabilitation for homeless persons and persons
at-risk of becoming homeless; and
WHEREAS, the U.S. Department of Housing and Urban Development has advised the County
that for FY 2025 it is eligible to receive the following entitlement amounts: $3,425,458 for the
CDBG Program; $572,270.43 for the HOME Program; and $299,886 for the ESG Program; and
WHEREAS, in addition to the entitlement hinds, the County anticipates receiving $50,000 in
CDBG program income; and
WHEREAS, the Redevelopment Authority of the County of Washington has prepared the FY
2025-2029 Consolidated Plan and FY 2025 Annual Action Plan which addresses the housing and
community development needs of County residents, especially extremely low- and low-income
households, homeless families and individuals, and others with special needs; and
WHEREAS, the County has prepared an amendment to its Citizen Participation Plan which
outlines the County's responsibilities for obtaining citizen comment on the Consolidated Plan,
Annual Plans, Plan Amendments, the Consolidated Annual Performance and Evaluation Report
(CAPER) and any Section 108 loan application should the County undertake one.

WHEREAS, a draft of the County's FY 2025-2029 Consolidated Plan and FY 2025 Annual Action
PIan was on display from May 12—June 10,2025,a draft of the Citizen Participation PIan was on
display from May 12 — 26, 2025, and the County has held public hearings on said Five-Year
Consolidated Plan,Annual Action Plan,and amended Citizen Participation Plan and the comments
of various agencies, groups and citizens were taken into consideration in the preparation of the
final documents.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COMMISSIONERS OF THE
COUNTY OF WASHINGTON:
1. That the FY 2025-2029 Consolidated Plan, FY 2025 Annual Action Plan, and amended
Citizen Participation Plan for the CDBG, HOME, and ESG Programs are hereby in all
respects approved and the Chief Clerk is hereby directed to file a copy of said Annual

2. That it is cognizant of the conditions that are imposed in the undertaking and carrying out
of community development, affordable housing, and emergency homeless activities with
federal financial assistance, including those relating to (a) the relocation of site occupants
and, (b) the prohibition of discrimination because of race, color, age, religion, sex,
disability, familial status, or national origin, and other assurances as set forth under the
certifications.
3. That the Chairman of the Board of Commissioners of the County of Washington,on behalf
of the Board of Commissioners, is authorized to file an application for financial assistance
with the U.S. Department of Housing and Urban Development which has indicated its
willingness to make available a $3,425,458 grant to carry out the CDBG Program; a
$572,270.43 grant to carry out the HOME Program; and a$299,886 grant to carry out the
ESG Program.
4. That the Chairman of the Board of Commissioners of the County of Washington, on behalf
of the Board of Commissioners, is hereby authorized to provide such assurances and/or
certifications as required by the Housing and Community Development Act of 1974, as
Amended, the National Affordable Housing Act of 1990, and the Stewart B. McKinney
Homeless Assistance Act of 1988, as amended, and also any supplemental or revised data
which HUD may request in connection with the review of this Application.
5. That the Chairman of the Board of Commissioners is authorized to execute on behalf of
the County the formal grant contracts and other documents relating to the County's FY
2025 CDBG Program, FY 2025 HOME Program, and FY 2025 ESG Program.
6. That the County Department of Human Services will administer the ESG Program.
7. That the County hereby designates the Redevelopment Authority of the County of
Washington as its agent for administration of the CDBG and HOME Programs and
authorizes execution of Subrecipient Agreements for both programs.

8. That the Redevelopment Authority of the County of Washington is authorized to execute
all required environmental review documents as the responsible entity agency on behalf of
the County.
